Competitor Payment Form 2011

In addition to completing the Competitor Nomination Form, a nomination fee of $330.00 must be made. Alternatively, the full competition fee of $1100.00 can be made. A Competition Nomination Payment Form must accompany payment.

Accepted forms of payment include any of three options:

• Credit Card (2% surcharge applies ) – Print and complete all four sections of the Nomination Competitor Payment Form, either fax the completed form to 1300 885 869 or mail to the address below.
• Cheque or Money Order – Print and complete sections 1, 2 and 3 of the Nomination Competitor Payment Form, attach “cheque” or “money order” then post to:
